| Definition |
A group of diseases characterized by weakness and variable wasting of affected muscles with involvement of both upper and Lower motor neurons, and without any sensory involvement; predominant involvement helps distinguish the specific variety. Primary Lateral sclerosis [UMN limbs] form [mixture of UMN &LMN limbs]. Amyotrophic Lateral sclerosis, the most common form [mixture of UMN & LMN limbs]. Progressive bulbar palsy [LMN; motor nuclei of cranial nerves in bulb], Pseudo-bulbar palsy (UMN bulbar motor nuclei & corticobulbar tracts). |

| Prognosis |
MND is one of the most devastating of all neuro-degenerative disorders with a mean survival of 3-5 years. It has figured on euthanasia or assisted suicide discussions. Patients with only LMN features have the best prognosis and may live up to 15 years. Those with bulbar involvement have have the poorest prognosis, and death follows pulmonary infection. Long-term ventilation after tracheotomy might be an unpleasant option. Care in a hospice might make terminal perios more comfortable. |

Physiotherapy.Psychotherapy for depression.Percutaneous endoscopic gastrostomy for dysphagis, preferably done earlyTracheostomy for respiratory paresis.Mechanical ventilation for bulbar failure. |
